SereNaWeb wird durch das Ministerium f\u00fcr Soziales und Integration Baden-W\u00fcrttemberg gef\u00f6rdert.<\/p>\n<p>Mit diesem Start entlassen in Baden-W\u00fcrttemberg bereits sechs Krankenh\u00e4user Ihre Patientinnen und Patienten digital \u00fcber caseform in die pflegerische Nachsorge. Die n\u00e4chsten Krankenh\u00e4user stehen schon bereit, womit der Einsatz von caseform in Baden-W\u00fcrttemberg schnell voran schreitet und das Netzwerk der beteiligten Nachversorger kontinuierlich w\u00e4chst.
